During your welcome call, one of our providers will get to know you and learn how we can best support you. If you need assistance connecting to this virtual session, we'll coordinate with tech support in your area to come by and help.
Personalized Specialists and Services
Depending on what care you choose, your care team may include a counselor, specialized medical providers, and community services that work together to get you back on your feet.
Counseling
Your therapist meets with you virtually or over the phone to hear you out and help you through your challenges, all from the comfort and convenience of your home.
Medication Management
A psychiatrist or psychiatric nurse practitioner can guide you to medication that fits your needs and total health– often decreasing previous medications when needed and mitigating reactions.
Connecting Care
A registered nurse and resource specialist work together to organize appointments, keep your provider in the loop, and offer extra support with community resources when needed.

What if I already have a good provider?
We don't replace your doctors; we work alongside them. Your care manager coordinates with your primary care provider and any specialists you already see, so your care feels more connected, not more complicated.
But how much is it?
We are in-network for traditional Medicare (including for the GUIDE program), Medicare-Medicaid dual-eligible, and many Medicare Advantage and Special Needs Plans. Depending on your plan and where you live, our services may be free or available for a copay. Before you get started, we’ll confirm your coverage so you won’t have any surprises.
