HOW WE WORK WITH PROVIDERS

A true partner for your practice

From the first referral through ongoing care, we work as an extension of your team so your patients feel supported and you stay up to date in real time.

Specialty support for your most complex patients

We specialize in supporting older adults living with complex behavioral health needs such as mental illness, substance use, dementia, and the cognitive and emotional shifts that come with later life.

Psychiatry and medication management

Our specialists understand aging brains and complicated medication lists. We review, reconcile, and send our prescribing notes back to you.

Talk therapy with licensed therapists

Evidence-based therapy for older adults as well as caregivers navigating depression, anxiety, grief, and life transitions. Patients can start with therapy alone if that feels like the right entry point or add at any time.

Wraparound care management

A registered nurse and resource specialist work together to coordinate appointments, support caregivers, navigate social needs, find additional resources, and serve as a steady point of contact between your office and ours.

How to get started

We handle the heavy lifting from the moment a referral comes in. You refer, and our team handles the rest: outreach, scheduling, insurance verification, setup support for patients who need help with technology, and follow-up to keep care on track.

step 01

Send the referral

Submit a referral through our online form or by calling. We also accept warm transfers and three-way calls if you have a patient you feel would benefit from Author Health in your office today.

step 02

We reach out to your patient

We handle outreach, insurance verification, and scheduling. For patients who struggle with technology, we send a community health worker to their home to help them connect to their appointments.

step 03

We care. You stay connected.

Your patient meets with a psychiatric provider for a comprehensive intake, then continues with their dedicated care team. You receive a consult note after the initial visit, each visit after, and ongoing updates on medication management.

step 04

We stay in touch

Reach out anytime to discuss a patient or flag something we should know.

We complement your care

Most caregivers didn't plan for this role. They just stepped up and kept stepping up. We’re here to help so you can keep showing up for your loved one and yourself, too.

We're a specialty extension of your practice, not a replacement for it. You stay at the center of your patient's care; we add the geriatric behavioral health expertise that's hard to find anywhere else.

What coverage looks like for your patients

Author Health is in-network for traditional Medicare (including GUIDE), dual-eligible plans, and many Medicare Advantage and SNP plans. We confirm eligibility before scheduling. Your staff doesn't need to.

Hear from your peers

We've earned the trust of primary care teams, specialty practices, and health systems across our markets. Here's what they say about working with us.

"Mild Cognitive Impairment can be concerning and confusing for patients and families as they experience cognitive changes while remaining mostly independent. This also serves as a window where people can plan for the future, change lifestyles, and pursue some new treatments. While there is not currently a cure for the disease, getting support could slow down the disease process and help caregivers and their loved ones experience less overall stress."

Dr. Ganesh Gopalakrishna M.D., M.H.A.

Senior Medical Director @ Author Health

“I’ve seen really good results with the model and support behind it. It takes a team, especially with how sick our patients are, how many co-morbidities they have and social determinants of needs. I think I’m doing right by my patients -- it all works, and it all works together.”

Michael Burrell

Nurse Practitioner @ Author Health
