Your privacy is important to Author Health, so we’ve developed this Privacy Policy to explain how weuse your personal information. When we use the term “personal information” in this Privacy Policy, wemean information about you that is personally identifiable to you.

In order to understand how we use your personal information, we need to explain Author’s legal organization because it affects how we use your personal information. As further described in our Terms of Service, Author Health, LLC operates the Site at authorhealth.com and other related websites and applications. An associated but independent clinical practice called Edge Health, P.C. (“Medical Group”) provides the licensed professionals who treat you on the Site. For purposes of this Privacy Policy, “you” means you on behalf of yourself and your Family Members. Both Author Health, LLC and Medical Group (collectively, “Author”, “we”, and “us”) may use and disclose your personal information collected through the Site, but how we use and disclose that information depends on whether it is health information or another type of personal information. For purposes of clarification, Author Health, Inc. does not provide any medical or clinical services.

We believe that your health information should have heightened privacy protections, which are also required by certain federal and state privacy laws. All collection, use, and disclosure of your health information through the Site is governed by Medical Group’s Notice of Privacy Practices. When you become a patient of Medical Group, you will be granted access to our patient portal via our third-party vendor Elation. We treat all health information that you provide when becoming a patient with us and via interacting with your Care Team as health information under Medical Group’s Notice of Privacy Practices. That means if we need to collect, use, or disclose that health information for any reason, we follow the Notice of Privacy Practices. Please review the Notice of Privacy Practices for more information on how we protect your health information and your rights under applicable laws.

This Privacy Policy explains how we collect, use, and disclose your other personal information that is not health information. By using the Site, you agree that we can collect and use your personal information as described in this Privacy Policy. If you do not agree, please do not use the Site or our Services. All capitalized terms in this Privacy Policy not defined herein shall have the meaning set forth in the Terms of Service.

The personal information we collect or maintain may include:

Identification Information. We track information that you enter into the Site (e.g., yourname, phone number, and email address when requesting more information about us andour Services, etc.).
Health and Medical Information: We collect such information in connection with providingsupport to you. All health information will be collected, used, and disclosed accordancewith the Notice of Privacy Practices.
Communication Information: We may collect information when you contact us withquestions or concerns and when you voluntarily respond to questionnaires, surveys orrequests for market research seeking your opinion and feedback. Providingcommunication information is optional to you.
Social Media Information: We have pages on social media sites like LinkedIn, Facebook,Twitter, and Instagram and anticipate being on other platforms in the future (“Social MediaPages”). When you interact with our Social Media Pages, we will collect personalinformation that you elect to provide to us, such as your contact details. In addition, thecompanies that host our Social Media Pages may provide us with aggregate informationand analytics regarding the use of our Social Media Pages.

As you navigate through and interact with our Site, we may use automatic data collection technologiesto collect certain information about your equipment, browsing actions and patterns, specifically:

Site Visit Details. We track details of your visits to our Site, such as traffic data, locationdata, logs, referring/exit pages, date and time of your visit to our Site, error information,clickstream data, and other communication data and the resources that you access anduse on the Site; and

Analytics Tools. We use certain third-party analytics tools to help analyze how individuals useand interact with the Site, including Google Analytics and Marchax. Certain features of the Site may, at a later date, use GPS technology to collect real-timeinformation about the location of your device so that the Site can connect you to a healthcareprofessional who is licensed or authorized to provide Services in the state where you arelocated.

In many circumstances, the information listed above is non-personal information. For example, theinformation collected through Cookies may not contain any identifiable personal information. We willstill treat that information as personal information under this Privacy Policy if (a) an applicable local lawconsiders it to be personal information or (b) we combine it with personal information.

De-identified information.

We may use de-identified information created by us without restriction. When we use the term “deidentified information,” we mean information that is neither used nor intended to be used topersonally identify an individual.

Children under age 18.

The Services are not available for use by children (under 18 years of age) and children are notauthorized to become Members and may not use the Services. Company does not knowingly collectpersonal information from children under the age of 18. If we learn that we have collected the personalinformation of a child under 18 we will take steps to delete the information as soon as possible to theextent permitted by law.

Marketing.

We may use your personal information to contact you to tell you about products or services we believemay be of interest to you. For instance, if you provide your email, we may use that information to sendyou special offers. You may opt out of receiving promotional emails by following the instructions

contained in each promotional email we send you. If you unsubscribe from our marketing lists, you willno longer receive marketing communications but we will continue to contact you regardingmanagement of your account, other administrative matters, and to respond to your requests. If youexpressly consent and provide us your phone number, we may use that information to text youmarketing messages. You can opt out of the marketing text messages you receive by contacting us at754 202 2655 or following the opt-out instructions in one of our messages.

Disclosure to third parties.

At times we may provide third parties with certain personal information to provide or improve ourproducts and services, including to help us market to consumers. When we do, we require those thirdparties to handle it in accordance with applicable laws. We do not sell personal information, andpersonal information will not be shared with third parties for their marketing purposes.

This Privacy Policy applies only to information we collect through the Site. This Privacy Policy DOESNOT apply to information collected by any third party. When you click on links on the Site you mayleave our Site. We are not responsible for the privacy practices of other sites, and we encourage you toread their privacy statements.

Modification of information.

Members will be able to update some of their information through the Site. Requests to modify anyinformation may also be submitted to contact@authorhealth.com.

Limitations on deletion of information.

You may request deletion of your personal information by us, however, we may be required (by law orotherwise) to keep this information and not delete it (or to keep this information for a certain time, inwhich case we will comply with your deletion request only after we have fulfilled such requirements).When we delete personal information, it will be deleted from the active database, but may remain in ourarchives (subject to applicable law) and we may also retain anonymous information about your use ofour services. Once we disclose some of your personal information to third parties, we may not be ableto access that personal information any longer and cannot force the deletion or modification of anysuch information by the parties to whom we have made those disclosures. After we delete personalinformation, we may retain de-identified information and will continue to use de-identified informationas permitted under this Privacy Policy.

Personal Information Processing in the U.S.

The Service is not intended for use outside the United States. By using our Service, you understand andacknowledge that your personal information will be transferred from your location to our facilities andservers in the United States, where data protection laws may differ from those in your jurisdiction.

Your Choices

In certain circumstances providing personal information is optional. However, if you choose not toprovide personal information that is needed to use some features of our Service, you may be unable touse those features. Right to non-discrimination.

Job Applications.

When you visit the “Connect With Me” portion of our website for Physicians, we collect the informationthat you provide to us in connection with your inquiry. This includes business and personal contactinformation, professional credentials and skills, educational and work history, and other information ofthe type that may be included in a resume. This may also include diversity information that youvoluntarily provide. We use this information to facilitate our recruitment activities and processemployment applications, such as by evaluating a job candidate for an employment activity, andmonitoring recruitment statistics. We may also use this information to provide improved administrationof the website, and as otherwise necessary (a) to comply with relevant laws or to respond tosubpoenas or warrants served on us; (b) to protect and defend the rights or property of us or others;(c) in connection with a legal investigation; and/or (d) to investigate or assist in preventing any violationor potential violation of the law, this Privacy Policy, or our Terms of Service.
