Am I eligible for GUIDE?
Your Author Health care team can help you assess your unique situation to determine if you meet the eligibility criteria for GUIDE. Call us, and we’ll help you get your benefits going.
You already have a diagnosis of dementia, or our clinicians see you fit the criteria for support
You have Medicare as your primary health insurance and are enrolled in Medicare Parts A and B (not Medicare Advantage)
You are not enrolled in a Medicare hospice benefit
You don’t reside in a long-term nursing home
You are not on another Medicare plan, including Special Needs Plans (SNPs) or Program of AllInclusive Care for the Elderly (PACE)

Caregivers need help, too
Dementia affects more than one person. Family members and caregivers are often the ones making calls, tracking changes, asking questions, and trying to hold everything together. Author Health’s GUIDE program offers caregiver education and support, and for eligible members, may also include respite services that give families a little breathing room.

Does my insurance cover GUIDE?
Author Health is proud to be one of the organizations across the US selected to participate in the Guiding an Improved Dementia Experience (GUIDE) Model. Enrolling in GUIDE gives extra resources for you and your caregivers–as part of your traditional Medicare coverage, at no additional cost to you.
