The GUIDE Model complements Author Health’s existing dementia care offering for traditional Medicare beneficiaries. Enrolling in GUIDE is beneficial because it unlocks additional resources for you and your caregivers–as part of your traditional Medicare coverage, at no additional cost to you.

Determining if you are eligible for GUIDE

Your Author Health care team can help you assess your unique situation to determine if you meet the eligibility criteria for GUIDE. We will help you navigate care for the program, and your GUIDE benefits will be seamlessly integrated into your personalized AuthorCare™ Plan.

Here’s a summary of the eligibility requirements:

  • You must have a diagnosis of dementia and this diagnosis must be confirmed by a clinician
  • You must have Medicare as your primary health insurance
  • You must be enrolled in Medicare Parts A and B (not Medicare Advantage)
  • You must NOT be enrolled in a Medicare hospice benefit
  • You must NOT reside in a long-term nursing home
  • You must NOT be on another Medicare plan, including Special Needs Plans (SNPs) or Program of AllInclusive Care for the Elderly (PACE)
